In order to connect your Satellite phone and/or to claim your Government Subsidy the following documentation is required.
 
Please take the time time download and fill out the following forms.

Additionaly should you wish to pay using direct debit, you will need to fill out a direct debit authorisation form.

Should you wish to port a phone number across to Globalstar you will also be required to fill out a Customer Authorisation Form.

Finally, If you have applied and been approved for Government Subsidy, please fill out the Subsidy Payment details form, nomintaing which account you would like us to credit the full subsidy amount into.

We will also require three forms of identification:

Individual ID Requirments:
1. Drivers Licence
2. Medicare Card
3. Credit Card EG. Visa, MasterCard.

Business ID Requirments:
1. Drivers Licence of Applicant
2. Purchase order on company letterhead (must have ABN on it)
3. Utilities bill with company trading address on it.


Once you have the three piece of Identification along with completed forms,
please post through to:

In order to speed up the process of your connection, you can also fax through the completed forms through to us on 1300 768 100, we can then begin processing, so your phone will be ready to use when it arrives. However we still require an origninal copy with your signature to be sent in the post.
